Dr. Jack R. Baker, a native of Cadiz, Ohio, attended Kent State University under a football scholarship and was graduated with honors in 1971. He then was awarded a degree in Physical Therapy from Ohio State University in 1972.

In 1980, Dr Baker graduated from the West Virginia School of Osteopathic Medicine. He  has completed two internships: a D.O. intenrship at the Delaware Valley Medical Center in Philidelphia, Pa. and a M.D. internship at Akron City Hospital, Akron, Ohio.

To specialize in Urology, Dr. Baker remained at Akron and completed a five-year M.D. residency in Urology in 1986.

Dr. Baker is National Urology Board Certification eligible and is an active member of the medical staff at Timken Mercy Medical Center
